I acknowledge that the wellness techniques and workshops offered by Corbin Chase are designed to assist individuals on their personal journey of self-healing and self-awareness. I understand that these complementary wellness techniques, including but not limited to Clinical and Transpersonal Hypnosis, Sound Vibrational Healing, Intuitive Psychic Counseling, Crystal Energy Healing, Reiki and Energy Healing, Guided Visualizations, Plant and Nature-Based Practices, Yoga, and Sauna and Infrared Light Sessions, are not a substitute for professional medical or psychological diagnosis and treatment.

Professional Qualifications Disclaimer

I acknowledge that Corbin is a certified wellness practitioner with expertise in various holistic and complementary wellness practices. A list of Corbin’s degrees, training, experience, credentials, and other qualifications is attached to this Form. While she holds esteemed certifications in these areas, Corbin is not a licensed healthcare professional, medical doctor or psychologist. Her services are intended to complement, not replace, the care provided by licensed healthcare professionals. These wellness sessions are designed to support individuals in their personal healing journeys. Participants are encouraged to consult with their licensed healthcare providers for medical or psychological advice or treatment.

Scope of Services

I fully understand that the services provided by Corbin Chase are holistic and complementary in nature. They are not meant to replace professional medical advice, diagnosis, or treatment. For any medical concerns, emergencies, or psychological issues, I agree to seek advice from a licensed health care provider or emergency services.

Release and Limitation of Liability

I voluntarily agree to assume any risk associated with the services I receive from Corbin Chase and/or Corbin Chase Health LLC. I hereby release and hold harmless Corbin Chase and Corbin Chase Health LLC, and any agents, associates or employees, from any and all claims, liabilities, damages, costs, or expenses that may arise directly or indirectly from my participation in the services, whether conducted on premises, remotely, or through any other venue.

Cancellation Policy

I acknowledge that there is a 50% charge for appointments or workshops not canceled or rescheduled at least 48 hours in advance.

Insurance Disclaimer

I acknowledge that Corbin Chase recommends maintaining appropriate health insurance and/or consulting with a healthcare provider where necessary.

In case of any immediate health needs, I understand that I should contact emergency medical services without delay.
